The spear gives logical explanation to all the abilities we have and we just cant use hidden blade+spear+ main weapon that would be too much and the spear is necessary from lore perspective so hidden blade just had to go for this game. But the difference is mainly visual the spear is still basically used like hidden blade. Bayek wasn't wearing the hidden blade wrong, he wore it the same way that Altair, Ezio, Connor, Shay, Arno, Jacob, and Evie wore their hidden blades.
